Why we created this additional service

After doing lots of CHIPs/Implementation Strategies throughout the years, we have observed that the CHIP (community health improvement plan) work plan often doesn't get done to its fullest potential and its progress loses momentum and energy. This is not because the assessment team does not care or doesn't want to see these changes; it is because of a whole plethora of reasons that include - confusion on the process of implementing the CHIP work plan, partner/staff turnover, not having the capacity/time/resources to do the tasks, and so much more. 

Over time, it has been our goal to find ways to reduce this and to support our clients with implementation of the CHIP so that it is successful and so that community health improvement truly happens in the years after the CHIP report is published. In our other services, we do that through a detailed work plan and discussing the plan for the implementation of the CHIP at the end of our contract (which traditionally has been soon after the CHIP/Implementation Strategy report is completed).

However, with our conviction because of our observations and the frustration we saw in our clients after the CHIP report was completed...we wanted to do more. After much consideration, thinking, and discussion, the CHIP Implementation Support service has been created.

What this extra enhanced support includes and how we support YOU

  • We support the CHIP work plan implementation through 1-2 quarterly progress meetings (equalling 3 hours total) EACH quarter for 1 year (totalling 8 max meetings for the year). Additional years of the phase can be contracted

  • We track accountability of the partners implementing the CHIP work plan

  • We keep the momentum and energy of the work plan progress going to successfully create community improvement

  • We plan and facilitate the quarterly progress meetings via video conference/Zoom

  • We share the responsibility of making sure work plan happens and alleviate stress of the CHIP team

  • We create and maintain the CHIP work plan used at quarterly progress meetings

Cost

  • $5000/year (minimum of 1 year) of the CHIP implementation part of the cycle

Requirements for this process to work

  • There must be a "keeper" of the work plan and each priority health need must have a “leader” that can speak to goals and report back on progress updates (these roles are usually already determined in the CHIP development phase).

    • "Keeper" of the work plan role: making sure work group meetings are scheduled; making sure leaders of work group know about quarterly progress update meetings, and attend the quarterly progress update meetings.

    • Priority health need work group leader role (ideal to have 2 co-leaders) of EACH group: scheduling and conducting work group meetings in advance of quarterly progress meetings (hold minimum of 1 meeting quarterly), being a spokesperson on goals and for partners, attending quarterly progress update meetings, and maintaining the list of work group members (and adjusting and inviting additional partners as needed)

  • A commitment from client(s) for community improvements and to this process! (We are all in and committed...so we want our clients to be also!)
